Once you schedule/reschedule your appointment, it will be sent to Darlene McDowell as a Request to Schedule/Reschedule. She will send you a confirmation of the new rescheduled appointment day/time, once approved. ** You may use this form if you have already paid for multiple services in advance. ..to make your future appointments. Fill in the form with your request for the day/time that is convenient for you. Darlene will email you back with confirmations, once approved. This is NOT for GROUPON. This is for website purchases only.
Cancellation Policy
